Life insurance underwriting classes — how to get the best rate class

I got quoted $45/month for a $500k 20-year term policy at 'Preferred' rates, but my friend with similar age and health got $32/month at 'Preferred Plus.' What determines whether you get Standard, Preferred, or Preferred Plus rates? Is it purely health-based, or do things like driving record, hobbies (I scuba dive), and family health history matter? Can I improve my classification before applying?
