Parent has dementia - how do I get power of attorney now?

My mom (76) was just diagnosed with moderate dementia. She can still communicate but her memory is declining fast. Doctor says she still has capacity to sign documents. Do I need to rush to get POA and health care proxy? How quickly does this need to happen?
